Our Medical Social Workers assist to resolve social, emotional and financial problems that might hinder the patient’s treatment or expected rehabilitation potential. They have routine conferences with home care staff and family members. They will also evaluate for possible relocation of residence and assist in placement in senior apartments, assisted living communities, and nursing homes.

Additionally, the Social Worker is there to help assess and determine any o the patients needs. This includes identifying any limitations, barriers or safety issues that could hinder recovery. The social worker is also responsible for educating the family on available community resources, support and assist with long range planning. These services can also include coordinating needs such as Advanced directives, assisting with emotional issues such as the loss of a loved one, crisis intervention, financial aid and counseling.
