Empowering Health with Personalized Nutrition and Care
With the right education from our staff, diabetes can be effectively managed in the comfort of the patient’s home. We offer personalized nutrition plans tailored to each patient, creating special diets that incorporate foods they enjoy. Our team also provides counseling on menu planning and managing nutritional needs.
Medical social worker
Supporting Recovery with Compassion, Resources, and Care
Our Medical Social Workers help address social, emotional, and financial challenges that may affect a patient’s treatment or rehabilitation progress. They regularly collaborate with home care staff and family members to ensure the best care possible. They also assess the need for potential relocation to senior apartments, assisted living, or nursing homes.
In addition, social workers evaluate the patient’s needs, identifying limitations, barriers, or safety concerns that could hinder recovery. They educate families on available community resources, provide support, and assist with long-term planning. Their services include coordinating advanced directives, offering emotional support during life transitions, crisis intervention, and providing financial aid and counseling.
